What is All-on-4?

All-on-4 is a tooth replacement system that integrates dental implants and fixed dentures. The tooth replacement system covers one whole arch — either all of your top teeth or all of your bottom teeth. If you need full mouth restoration, you can get two All-on-4 systems, one for each arch.

All-on-4 can be an excellent alternative to other types of tooth replacement options. It's a convenient and fast way to replace all of your teeth. In most cases, you can get a whole new row of teeth in just one appointment. All-on-4 can give you permanent teeth at a lower cost than full dental implants.

What is the process of getting All-on-4’s?

Although you’ll need only one surgery for All-on-4, there are a few preparatory steps to make sure your restorations are perfect. 

Dental exam

The first step in the process is a dental exam. Dr. Stern evaluates your overall oral health and determines the best tooth replacement options for you. 

CT scan

You’ll usually have a cone beam computed tomography (CBCT) scan, which provides 3-D images of your jaw. Dr. Stern uses these images to determine how much bone you have to support dental implants, where your facial nerves are located, and other essential information that he uses in treatment planning.

Computerized implant simulation

Dr. Stern uses an advanced computer simulation to design your new fixed bridge and plan your dental implant placement. He designs the optimal surgical plan based on the implant positioning that provides maximum support for your fixed bridge. 

All-on-4 oral surgery

You’ll receive local anesthesia for your oral surgery. During the procedure, Dr. Stern creates small gum incisions to place the four dental implants into your jawbone. Usually, you’ll have two angled implants in the back of your arch and two straight implants in the front.

Dr. Stern attaches the fixed dentures to the dental implants securely. Then, you’ll go home with a full arch of new teeth. 


The entire All-on-4 surgery takes around 2.5 hours per arch. 

Am I a good candidate for All-on-4?

Dr. Stern can discuss your candidacy with you in detail. In general, most people are good candidates for All-on-4. 

While traditional dental implants require significant bone density, All-on-4 is more flexible because it only utilizes four implants. Dr. Stern can choose the optimal areas (those with the most healthy bone) for the implants. You may even be a good candidate for All-on-4 if you have gum recession.
